Ready for Home?: Improving hospital discharge care for people living with COPD
This report from the British Lung Foundation and the British Thoracic Society was published in December 2010. It summarises the results of two surveys (of patients and clinicians) about the current arrangements for preparing patients who have been admitted with an exacerbation of COPD for discharge to home. The surveys reveal that much more needs to be done. BLF and BTS intends to produce more work in this area during 2011.
