IMPORTANT: ERROR IN FLOW DIAGRAM: Appendix 3, page i37. After the question "Does the patient have pneumonia?", the flow chart incorrectly directs the reader down the left-side for the management if the answer is yes, and down the right hand side if the answer is no.
